Behavioral Wellness Coach-HCBS On Call Bronx NY US About the Position The position is primarily responsible for rendering services that are outlined on Child s Care Plan to children and youth who have developmental medical or other health needs. Duties & Responsibilities o Ensures the timely and quality execution of services outlined in each child s Care Plan. o Collaborates with stakeholders including the Care Coordinator Supervisor (who manage service planning for program enrollees) to advance the achievement ofoutlined service goals. o Documents delivered services as required by the program (i.e. Progress Notes and other forms outlined by the program.)
