POSITION DUTIES Maintain accurate detailed reports and records of patients at skilled nursing facility. Discuss Treatment plan and prescribe medications with patient family and other health care workers. Administer medications to patients and observe patients reactions or side effects. Record patients medical information lab test results and vital signs. Monitor record and report symptoms or changes in patients condition. Consult and coordinate with healthcare workers to assess plan implement or evaluate patient care plans. Modify patients treatment plans as indicated by patients responses and conditions. Observe all aspects of patient care including diet and physical activity. Prepare patients for and assist with examinations or treatments.
