Licensed Social Worker Care Manager Family Service of Lucas County a service location of Family Service of NW Ohio is looking for a LSW to fill the Care Manager position. This full time position provides overall care management including comprehensive assessments and care plans that include the client s physical health behavioral health and social service needs. Minimum of Ohio LSW or PC is required Master s degree preferred. Must be at least 21 pass background check & post offer pre-employment drug screen have a valid driver s license acceptable BMV report dependable transportation and proof of auto insurance.
