Admission Social Worker - Compensation: DOE

Responsible as a member of the interdisciplinary team to provide information about Hospice of Saint John services to potential patients and to assure smooth admission process of patients to the HOSJ hospice program. Essential Job Functions Responsibilities 1. Complete admission of patients into the Hospice of Saint John hospice program in coordination with the Referral Coordinator and Referral Nurse. 2. Work with Referral Nurse to assure patients meet guidelines for hospice appropriateness. 3. Initiate plan of care with the nurse. 4. Assure data regarding physical psychological emotional and spiritual issues that may influence patient family conditions is obtained and incorporated into the plan of care. 5. Instruct and educate patient and family about scope of hospice services. 6. Perform initial psychosocial assessment and create and implement plan of care to meet patient and family needs. 7. Offer resource procurement and referral services as appropriate. Assist with placement as needed or appropriate. 8. Provide information regarding new admissions to the direct care team. Does not need to attend meetings but needs to communicate as soon as possible with the Interdisciplinary Team. 9. Assist the members of the IDT in understanding significant social and emotional factors related to the patient s health status. 10. Maintain proper records of contacts with potential patient and families. 11. Assure completion of all admission paperwork and delivery to Medical Records. 12. Assume the active role of advocate for the patient family unit. 13. Provide short-term crisis intervention when indicated. 14. Participate in community education and outreach programs as requested. 15. Adhere to the privacy practices as described in the Notice of Privacy Practices and as required by the privacy rule of the administrative simplification provisions of the Health Insurance Portability and Accountability Act of 1996. 16. Observe safety precautions when in the office and out in the field. Reports all accidents and incidents observed. 17. Attend all required meetings or obtains approval for absence when necessary. 18. Seek to improve professional skills by attending seminars and other training opportunities. 19. Share in providing 24-hour seven day a week coverage to patients and families when necessary. 20. Practice positive and consistent teamwork and communication. 21. Perform all duties in compliance with Hospice of Saint John policies and procedures. 22. Perform other duties as requested by supervisor. Position Qualifications Bachelor s Degree in Social Work from an accredited college or university. MSW preferred. Demonstrated ability in casework counseling and group work preferably in a hospice setting. Working knowledge of the Hospice Medicare Benefit. Demonstrated experience in presenting an explanation of hospice benefits and services. Commitment to the philosophy goals and objectives of the hospice concept. Display excellent written and oral communication skills. Able to rely on technical knowledge and problem solving skills to plan and accomplish goals. Valid Colorado Driver s License and reliable transportation on a daily basis.
