Nursing Facility Transition Coordinator

Disability Network of Mid-Michigan (DNMM) a non-profit organization advocating for peole with disabilities is hiring an individual to work part-time (up to 24 hours per week) as a Nursing Facility Transition Coordinator. The primary function of this position is to coordinate the following activities Assess consumer needs to determine supports that will be necessary for a successful transition Offer and facilitate person-centered plans with consumers Provide information on housing and community-based resources Assist consumers in their move to the community and provide follow-up services. The ideal candidate will have experience with advocacy and disability knowledge of person centered planning self-determination as it relates to people with disabilities knowledge of nursing homes and health care guidelines and be experienced in accessing community based resources. A background in the field of social work or other human service related field is preferred.
