Click here for more information about Health Care Agency THE DEPARTMENT Adult Correctional Health Services (ACHS) provides medical dental nursing infection control health education and pharmaceutical services at a community standard of care to all adult inmates in the County s five correctional facilities and contracts with hospitals for inpatient and specialty care. ACHS also provides mental health services including 24-hour emergency triage and crisis intervention suicide prevention treatment beds individual and group therapy substance abuse counseling medication assessment and discharge planning to all adult inmates in the County s five correctional facilities. GENERAL DUTIES Under direction with the ACHS Administrative Nurse Practitioner and Medical Director the Comprehensive Care Nurse Practitioner II general duties are as follows Provide chronic and urgent care to patients Provide case management for chronic diseases Provide care to patients with HIV and other types of sexually transmitted diseases Obtain complete medical histories of physical development illnesses injuries and treatments including obtaining medical records from outside health care providers Collaborate closely with onsite physicians to assess and manage difficult cases Collaborate closely with onsite RNs to ensure appropriate levels of care delivery Collaborate closely with Mental Health providers to ensure appropriate treatment of complex patients Order and perform diagnostic tests required by Agency policy and patient needs Perform complete physical examinations analyze data and determine diagnosis Develop implement and monitor patient treatment plans Assist in planning implementing and participation in special projects Serve as a liaison with other programs and departments in reaching satisfactory solutions and interventions while providing a high quality of professional care Other work as assigned HOW TO APPLY Please go to our website at www.ocgov.com jobs and submit your application online.
