RN Nurse Manager Care Navigator

We are in need of an experienced RN Nurse Coordinator Care Navigator for a clinic setting located in Winston-Salem. This position will require coverage Monday through Friday from 8 00am to 5 00pm with an occasional need to work some Saturdays to meet with special care patients. Regular staff and managerial meetings after hours may also be needed. Duties will include quality patient care by identifying their needs creating effective plan of care working with Providers and Staff to accommodate care needed for special needs patients. Must have outstanding communication skills in order to communicate with Provider Staff Source Representatives co-workers and patients. Able to move quickly think outside of the box a lot of regulatory work and documentation well versed in utilizing computer related applications and equipment. Must be a graduate of an accredited school of Nursing program with preference as BSN with prior case management and RN experience. Bilingual candidate would be a plus.
