RN Care Coordinator (HCO)

Family Service of Northwest Ohio Home Care Options is seeking a high energy top-notch RN to conduct in-home health social assessments and coordinate care for elderly clients. Awareness of community resources needed. Ability to work independently detail oriented comfortable working in the homes of elderly frail and disabled population possess OH RN license in good standing and experience in home health care medical social work gerontology or related field required. Local travel required. Applicants must be at least 21 pass background check & post offer pre-employment drug screen & physical with a valid driver s license acceptable BMV report dependable transportation and proof of auto insurance.
