REPORTS TO THE RN CLINICAL COORDINATOR SUMMARY The Case Manager is a qualified professional responsible for the coordination and delivery of quality home health care to the patient in accordance to the State and Federal regulations. ESSENTIAL JOB FUNCTION RESPONSIBILITIES 1. Instruct and guide clinicians to promote effective performance and deliver of quality home health care. 2. Ensure that the services provided are coordinated with the physician and other ancillary personnel. 3. Ensure that the frequency of visits are being followed as per plan of care. 4. Coordinate utilization review along with the RN clinical coordinator to ensure that there is no apparent over utilization or underutilization of services. 5. Generate infection control patient incident grievance report as needed. 6. Perform patient satisfaction surveys on all patients assigned. 7. Generate telephone orders communication notes and transfers under the supervision of the nursing supervisor. 8. Assume additional responsibility as designated by the Director of Patient Care Services. Job Requirements 1. Currently licensed though the State Board of Nursing 2. At least one year clinical experience consistent with the services that are provided by Premium Home Health Inc. 3. Must possess good nursing judgment and excellent oral and written communication skills. 4. Complies with acceptable professional standards and principles. 5. Capable of working with minimal supervision resourceful and with good organizational skills. 6. Flexible and cooperative in fulfilling role obligations.
