Qualified Health Home Specialist

Family Service of Northwest Ohio - Lucas County is currently recruiting a Qualified Health Home Specialist to be part of the Health Home team. The Health Home team coordinates physical and behavioral health and social services for eligible individuals thus providing a coordinated person-centered system of care. The Qualified Health Home Specialist assists with care coordination referral linkage and follow-up as part of the Health Home team. A Bachelor s degree in a social service related field is required. Must be at least 21 pass background check & post offer pre-employment drug screen have a valid driver s license acceptable BMV report dependable transportation and proof of auto insurance.
