Omaha Health Start (OHS) an initiative of Charles Drew Health Center (CDHC) is looking for a Social Work Case Manager (SWCM) with a masters degree in social work to join our staff. The incumbent will be responsible for providing social work case management services for high-risk women infants and children up to two years of age participating in the OHS HealthNET program. The SWCM will work in collaboration with a nurse case manager to address all risks associated with pregnancy chronic disease management health education and training and mandated quarterly nursing assessments. The SWCM is responsible for providing culturally sensitive coordinated client assessments referrals monitoring and facilitation as well as follow-up services for OHS case management participants. The position will also provide behavioral health assessments referral tracking and reporting for all program participants. The position specific competencies and essential functions duties and responsibilities are Provide psycho-social case management services for OHS participants in a variety of settings to include referrals for health and social services needs Work in partnership with the nurse case manager outreach staff and education coordinator Coordinate behavioral health assessments tracking and reports for OHS HealthNET program participants Determine eligibility for Medicaid Managed Care pregnancy test purposes the State SCHIP program Kids Connection CDHC sliding fee medical services and all other sliding fee based programs Utilize the existing health and social services organizations serving families in East Northeast Omaha for the purpose of initiating client referrals Develop a plan of service with the participant in partnership with the OHS nurse case manager medical provider health and social service referral sources and or families as are appropriate Provide ongoing assessment of needs and progress toward successful transition through feedback from the participant referral sources and nurse case manager Input data and documentation for case files into administrative dataset per OHS reporting guidelines and expectations Provide assistance as needed in preparing monthly performance indicator reports for OHS Provide assistance as needed with the OHS Incentive program and Other duties as assigned. The incumbent should have experience in consumer advocacy case management working with diverse racial and ethnic groups data collection records maintenance and care coordination. Good oral and written communication skills are necessary as well as being a self-starter strong organizational skills flexible and being able to multi-task and required. Spanish speaking is helpful must be willing and able to travel in and out of state and possess valid State of Nebraska driver s license and own reliable mode of transportation.
