Case Manager

Purpose of Your Job Position The Case Manager C C Is a professional who coordinates with the interdisciplinary team resident family on complex aspects of the residents care and serves as the focal communication point for residents and staff. C C Works closely with the attending physician(s) social services nurses and other members of the team throughout the continuum of care for identified complex in-patients. C C Is a clinician who in conjunction with the interdisciplinary team is accountable for assessing the complex plan of care effective and efficient utilization of resources meeting established professional and regulatory standards and who collaborates with the healthcare team and the resident family to accomplish agreeable outcomes. C C Works with new admissions assessing their conditions and needs in order to develop personalized care plans. They regularly evaluate these plans of care and update them on an as-needed basis. C C Works with physicians staff from other departments third party payors HMO s families etc. to facilitate the transition of the resident throughout the continuum at the appropriate level of care. Qualifications and Education Requirements Continuing Education As appropriate to maintain current knowledge and skill. Must show evidence of continuing education in management leadership seminars and workshops. Experience Experience applying customer service behaviors and communication skills required. Minimum 2 years experience required. Must demonstrate excellent clinical knowledge and be able to perform assessments and evaluate services and display excellent interpersonal and leadership competence in order to interact effectively with the resident the resident s family community service providers agencies attending physician(s) other healthcare team members payors etc. Must demonstrate the ability to prioritize workload and manage time efficiently. Certifications Licensure Current California licensure to practice professional nursing required (LVN RN) certification in case management field preferred. Current CPR Certification required. C Must have current and on-going working knowledge of the Medi-Care and Medi-Cal process. Dual eligibility HMO contracts. C Must possess a thorough knowledge of principles of effective communication mass media advertising policies education organization structure social and activity services government relations etc. as they relate to Marycrest Manor s operations. C Computer literacy in Windows and Web based programs including but not limited to all Microsoft Office programs. C Fluently read write speak and understand the English language. C C Must maintain open and clear communication and relations with Administration and Business Office. Must possess the ability to deal tactfully and professionally with personnel residents family members visitors government agencies personnel and the general public. Duties and Responsibilities Assists with the development revision and coordination of the Interdisciplinary Plan of Care for complex discharges through communication with physician social services nurses and other health care team members to meet the residents needs effectively efficiently and at the appropriate level of care required. Works with physicians staff from other departments third party payors HMO s families etc. to facilitate the transition of the resident throughout the continuum at the appropriate level of care. Coordinates complex discharge services throughout the continuum communicating pertinent information of residents progress and plan of care to appropriate referral sources alternative levels of care and community agencies. With other team members and information from the chart assesses the complex residents needs their support systems and available community service providers and assists with appropriate and cost-effective discharge planning. Documents actions appropriately. Serves as a resource contact and information source to residents families significant others and staff. Provides suggestions and collaborates with the development of new programs and services or revision of existing processes based on data analysis of length of stay variances readmissions scheduling and practice patterns DRG and cost management data and other sources. Contributes to the formulation and updating of unit policies procedures and protocols and adheres to these and other professional and regulatory standards. Through measurement and trending of resident outcomes and various data sources identifies areas of performance improvement and opportunities. Follow all safety procedures and regulations established in the employee handbook. All other duties as assigned by the Administrator. Working Conditions C Fast paced work environment C Is subject to frequent interruptions. Must be able to interact with emotional situations that may have behavioral issues. Communicates with nursing personnel department managers discharge planners etc. Is subject to be called back during emergency conditions (e.g. severe weather evacuation post-disaster etc.). Skills and Job Requirements Project a professional image through in-person electronic and phone interactions Demonstrate positive attitude and enthusiasm in support of Marycrest Manor s goals policies and principles. Be a supportive team member contribute to and be an example of teamwork and team concept. Excellent working knowledge of Microsoft Office (Word Excel ). C C Ability and willingness to work as an individual and as part of a team in the planning development and implementation of projects supportive of administration. Must have experience in working with highly difficult situations. Must maintain a positive disposition composure at all times. Ability to communicate with others in a timely and calm manner. Ability to work harmoniously with other personnel. Physical and Sensory Requirements Must be able to move intermittently throughout the workday and subject to frequent interruptions. Must be able to speak see and hear with or without accommodations. Must be able to push pull move a minimum of 25 pounds and be able to push pull move such a weight a minimum distance of 50 feet. Must be able to lift and or carry a minimum of 25 pounds and lift to a height of 5 feet.
