This experience should be immediately prior to employmentNo major preventable accident in the past three years (a major accident is defined as a DOT reportable accident).No more than two minor (non-DOT) preventable accidents in the past three years.Must have a valid CDL issued by the state of residence and have no more than three moving violations in the past three years. No serious traffic violations as defined by Section 383.5.No DUI DWI reckless driving hit and run or drug related convictions in the past five years.No positive drug tests or alcohol tests results of 0.02% or greater in the past five years.No one currently on probation or parole.Must be able to pass a U. S. Department of Transportation physical examination as required by Section 391.41 and successfully complete a pre-employment substance abuse test as required by Section 382.301.Must complete the company orientation class.Must not be addicted to use sell deal or transport controlled substances of any kind.No felony convictions in the last 10 years. All other criminal history convictions must be reviewed approved.Disqualifications may result from but not be limited to the following items Any major preventable accident (DOT reportable definition).Two preventable accidents in a 6 month period or 3 preventable accidents in a 2 year period.No more than 2 moving violations in a 12 month period and no more than 3 moving violations in a 3 year period.Conviction of any major DOT violation-leaving the scene of an accident speeding over 15mph driving recklessly improper erratic lane change following too closely operating a CMV without a valid CDL being under the influence of drugs or alcohol refusing a drug or alcohol test causing a fatality while violating traffic control laws or using a CMV to commit a felony.Any bond forfeiture or conviction of driving while and or under the influence of drugs and or alcohol unlawful possession of a controlled substance. Use of a controlled substance unless prescribed by a physician (providing it does not interfere with your ability to safety operate a commercial motor vehicle). Drinking intoxicants or being under the influence thereof while at the wheel or in the proximity of company equipment or equipment leased to the carrier eight hours prior to reporting for duty on company property for any reason or the property of any customer or while being required to be in readiness for dispatch. Having possession of alcoholic beverages either open or unopened or containers that have contained or containing alcoholic beverages within the tractor.Any preventable claim theft of any cargo or negligence that results in the loss of a load and or equipment.Any tampering removal sale or negligence of any equipment or parts owned by or under the control of the carrier.Transportation of a passenger without prior written authorization.Any conduct that is unbecoming of a driver. Examples fighting disorderly conduct poor customer relations drunkenness or other conduct that could jeopardize the liability of the carrier.Failure to pass or refusal to submit to an alcohol and or substance abuse test as required by the U. S.
