Care Coordinator

The Care Coordinator is responsible for ensuring the provision of direct services and service coordination to qualified individuals. This includes developing and updating Plans of care monitoring services in the plan doing regular visits with individual providers and other direct supports. The Care Coordinator is expected to meet the requirements established by Senior and Disability Services (SDS) as outlined in the SDS Care Coordinator guidelines of the Provider Packet. Education & Experience Preferred Bachelor s degree in social work human services psychology or other related field with one year direct care experience or three years direct care experience preferred. Prior study in or work experience with developmental disabilities is preferred. Required High School diploma must have current care coordination certification or obtain within the first 90 days of employment. Must have a valid Alaska Driver s license continuous use of a vehicle required level of automobile insurance must be able background check and TB test CPR First Aid certification.
