Family Service of Northwest Ohio - Lucas County is looking for a Licensed Social Worker or Licensed Professional Counselor to fill the Care Manager position. This full time position provides overall care management including comprehensive assessments and care plans that include the client s physical health behavioral health and social service needs. Minimum of Ohio LSW or PC is required Master s degree preferred. Must be at least 21 pass background check & post offer pre-employment drug screen have a valid driver s license acceptable BMV report dependable transportation and proof of auto insurance.
