RN Care Coordinator (HCO)

Family Service of Northwest Ohio Home Care Options is looking for a Registered Nurse to conduct in-home health social assessments and coordinate care for elderly clients. Awareness of community resources needed. Experience in home health care medical social work gerontology or related field preferred. Applicants must be at least 21 pass background check & post offer pre-employment drug screen & physical with a valid driver s license acceptable BMV report dependable transportation and proof of auto insurance.
