Clinician - Behavioral Health Navigator (IL)

Collaborate with Behavioral Health Navigator (BHN) to be the initial point of entry to the continuum of behavioral health services at Sweetser and within the community. The BHN will partner with the patient and the Primary Care Practice to manage the transition points between the Primary Care Practice and the Behavioral Health Services at Sweetser and within the community. This may include but not be limited to completing a full comprehensive assessment to identify the patients treatment needs and managing the referral to the appropriate level of care. JOB REQUIREMENTS Actively participate in analyzing the results of behavioral health screening tools. Demonstrate competence in quick and accurate clinical assessment of behavioral health conditions. Demonstrate competence in coordinating with primary care practices to manage referrals for patients with behavioral health conditions. Provide information or education about services and explain the benefits of Integrated Behavioral Health services. Demonstrate an ability to communicate key facts concerns and options to patients and other team members in both formal and informal opportunities. Demonstrate an ability to manage the pace of primary care and working with an interdisciplinary team. Demonstrate ability to implement clinical pathways and protocols for treatment of certain chronic and or co-morbid conditions. Demonstrate the ability to effectively evaluate crisis situations and employ appropriate interventions. EDUCATION REQUIREMENTS Master s degree in Social work Independently licensed to practice as an LCSW in MaineEXPERIENCE AND OTHER REQUIREMENTS Minimum of 2 years experience in the social work field Experience working with individuals of all ages including children and their families Demonstrated knowledge of the trend toward Patient Centered Medical Homes co-location of behavioral and physical healthcare and specific behavioral health related issues such as trauma substance abuse and recovery oriented care. Must possess and maintain State of Maine driver s license
