Clinician - Intake Specialist

Clinical intake specialist oversees and completes the processing screening coordinating and triaging of incoming referrals for placement with residential and day treatment program from referral screening and intake interview process for client admission.ESSENTIAL FUNCTIONS Receive and process referrals for services residential and day treatment Review referral records as needed to determine appropriate services to meet client needs. Completes clinical screening with parents guardians and providers and completes assessment tools Leads the clinical interview Complete necessary documentation for all referrals in accordance with department and organization expectations. Writes the clinical assessment with recommendations and diagnosis Obtain appropriate clinical records in preparation for assessment services. Provide information on all organization services to internal and external customers as necessary. Completes APS information including authorization requirements for APS Coordinates the admission Helps to develop school plans including attending IEPs Collaborates flexibly and responsibly with the area service network on behalf of clients to support the smooth access to the program coordinated service responses and client linkage with needed community resources and promote integration of care Be knowledgeable about available community resources for children families and adults on a statewide basis. Assist with resource coordination as appropriate. Attends timely transitions at the local hospitals and interacts at the program level with OCFS and other agencies at the State level. Responds to requests for technical expertise and or support from clients family members community based service providers and state agencies in order to support the admissions of children to the program Provides leadership and promotes positive solutions in staff clinical and administrative meetings Provides program supervision education clinical direction and support to clinical staff in interviews and in the intake process but possibly in other clinical areas such as assessment interventions and documentation to ensure the delivery of the highest quality of care the maintenance of a safe and therapeutic service environment and the maintenance of client dignity and confidentiality in all service delivery. Assigned a small clinical case load Participate in regular supervisory sessions with supervisor. Reports to the Director of Clinical servicesEDUCATION Master s degree in a professional clinicial discipline requiredCERTIFICATES LICENSES REGISTRATIONS REQUIRED LCSW LCPC LMFT - Independent licensure as a clinical mental health professional preferred. Valid state driver s license.EXPERIENCE Strong clinical skills in assessment treatment interventions and documentation Computer skills and navigation of the electronic record Experience with APS funding preferredKNOWLEDGE AND SKILLS Assessment diagnosis and clinical interventions Strong knowledge base in systems family therapy
