Currently we are having the requirements for Registered Nurses(Case Manager) at one of our clients office here in Detroit. Here are some of the job Requirements and Responsibilities The case manager will assess plan implement coordinate monitor evaluate and advocatethe options and services required to meet individual s health needs. Assisting members in meeting and managing both health care and quality needs. Establishing measurable goals which promotes evaluation of the cost and quality outcomes ofthe provider. As a patient advocate seek authorization for case management from the recipient of services Setting goals and time frames for goals appropriate to the individuals. Maintain communication and collaborating with the patients family physicians and health teammembers and payer representatives. Education and or Experience Registered Nurse with current Michigan License required Nursing Diploma or Associate degree in Nursing Bachelors degree in nursing of related fields (preferred) CCM certification (preferred). If not upon hire encouraged to become CCM certified with in 4Years of functioning in case management role. Work knowledge of Case Management principles and procedures based on nationally recognized standards of Case Management. 2-4 years of clinical experience preferably in Case Management or Home Health Care with a Medical Surgical background.
