We are actively recruiting a dedicated Registered Nurse to join our client s Home Health Care team. As a Home Health Care RN you will play a crucial role in providing comprehensive patient care through assessment planning and the delivery of direct nursing services while adhering to the agency s policies and standards. This position reports to the Clinical Manager.Required Experience Current licensure as a registered nurse in the State of Michigan. Highly preferred A minimum of two years of nursing experience in an acute care or home care setting. CPR certification is a plus. Demonstrated compliance with accepted professional standards and practices. Ability to work independently with minimal supervision. Strong team player. Responsibilities Take accountability for a designated caseload of patients and develop personalized care plans. Ensure strict adherence to privacy and security standards mandated by federal state and local authorities including the Health Insurance Portability and Accountability Act (HIPAA) and relevant state laws. Deliver care treatment and services in alignment with professional standards of practice practice guidelines laws and regulations. Develop and implement comprehensive patient-centered care plans for each individual updating them as per physician-written orders while fostering patient collaboration and understanding. Conduct planned home visits in accordance with the individualized patient Plan of Care. Facilitate referrals to other necessary services as indicated. Maintain accurate clinical documentation following agency policies and accepted nursing standards. Fulfill any additional duties as assigned.
