Huge Sign-On Bonus Searching for a dedicated and experienced Registered Nurse (RN) to join a Home Care team. In this role you will provide personalized and compassionate in-home care with a primary focus on wound care and rehabilitation for our patients. THIS POSITION IS IN BROOKLYN Requirements Bachelor s degree or higher in Nursing. Valid Registered Nurse (RN) license certification in the state of New York (NY). Minimum of 2 years of nursing experience in a related field. Specialized experience of 2 years in either Medical-Surgical (Med-Surg) or Home Health (Home Care). Key Responsibilities Provide direct patient care in accordance with policy goals objectives philosophy and Standards of Nursing Practice. Administer wound care manage IVs and catheters and administer medications. Perform assessments of patients physical psychological and social needs and create effective care plans. Collaborate with the interdisciplinary team to ensure holistic patient care. Schedule and conduct patient visits including start of care and revisits. Document all services provided maintaining compliance with government regulations Health policies and procedures. Participate in regional team activities including orientation support and performance feedback. Communicate effectively with team members management and regional leadership. Travel to patients homes to deliver care adhering to established schedules and protocols. Uphold the principles of patient advocacy ensuring patient rights and dignity are maintained at all times.
