They are committed to helping individuals access eligible federal state and municipal benefits ensuring they receive the comprehensive care they deserve.Position Overview As a Nurse Practitioner you will play a pivotal role in their mission to provide holistic care to individuals with Serious Mental Illness. You will be responsible for conducting whole-person clinical assessments leading the medical component of team-based care and facilitating care coordination. We are seeking a compassionate and experienced Nurse Practitioner with a deep commitment to improving the lives of underserved populations.Key Responsibilities Perform comprehensive clinical assessments including gathering information from external sources assessing both behavioral and physical health conditions and capturing undiagnosed conditions through thorough history and physical examinations. Organize assessment information to support disability applications when applicable. Conduct visits in the field or in individuals homes as needed. Make individualized recommendations and referrals for programs and care provided by both the company and outside providers based on assessed needs. Lead the medical component of team-based care providing training and guidance to Guides and Community Resources Guides on clinical issues and treatments. Support the Guide Team in care coordination efforts. Identify actionable care gaps and participate in care management. Perform medication reconciliation post-discharge and as needed. Communicate and coordinate with individuals care providers primarily at CMHCs FQHCs and hospitals. Qualifications Master s degree or higher in Nursing. Valid Nurse Practitioner (NP) license certification in Michigan (MI). 3 years of experience in one or more of the following areas Primary Care Value-Based Care. HCC Coding experience preferred. Empathy compassion and approachability. Excellent listening communication and interpersonal skills. Desire to help underserved populations on their path to wellness. Ability to work collaboratively as part of a multidisciplinary team. Cultural humility knowledge of trauma-informed care and social determinants of health. Comfort with data-driven care. Familiarity with local healthcare providers. Ability to maintain professional boundaries and engagement skills in non-traditional work conditions. Intermediate computer skills including familiarity with Microsoft Office Suite. Valid driver s license private auto insurance and insurability under coverage. Experience Experience working with and managing underserved populations with challenges such as behavioral health and or Substance Use Disorder (SUD) conditions. Experience in developing and delivering educational programs to nurses and affiliated medical staff. Prior experience in care management and coordination is highly desirable.
