Registered Nurse

We are seeking a dedicated and experienced Registered Nurse (RN) in Tucson AZ as a Care Manager. As a Care Manager RN you will play a pivotal role in creating and administering care plans focusing on the prevention and management of kidney-related health issues. This is not a direct clinical services role but involves comprehensive care coordination and support for patients particularly those with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD).Work schedule Monday to Friday 8 AM - 5 PM with 3-4 days on the road visiting patients and 1-2 days telehealth.RequirementsGraduate of an accredited School of Nursing. Current RN licensure in the State of Arizona or a Nurse Licensure Compact (NLC) state. Minimum of 2 years of previous experience in care management and or working with CKD ESRD patients preferred. Home Health and or Hospice experience is required. Ability to travel within a one-hour radius for in-home visits. Strong communication and interpersonal skills.
